What do I do if I’m in crisis?

If you are in crisis where you or someone else in danger, call 911. Call our 24 hour Inpatient Unit at 1 (800) 342-5653.

What information is required to make an appointment?

We will need the client’s date of birth, Social Security Number, address and insurance information.


What should I bring to my assessment?

Please bring your picture ID and completed information packet that was mailed to you. If you did not receive an information packet, you can download it here.
